After you make an appointment for an in-person or virtual visit, you will be sent a series of forms to complete, including a comprehensive Menopause Questionnaire, a Medical History Form and a series of consents to complete. It helps to return them at least 30 minutes prior to our meeting time.
I will review them before we speak and then again with you during our first visit. Then depending upon your questions, most pressing concerns and your goals, explanations will be provided and treatment options explored to enable us to work together on an individualized plan for your health.
You’ll also be provided a Menopause Symptom Tracker and a copy of The Menopause Guidebook from the North American Menopause Society.
